EMERGENCY WAIVER

By sending this electronic form to La-ti-Da Music, I, as the parent, Agency Representative, or Legal Guardian for the child designated in this form, hereby hold La-ti-Da Music harmless for any and all damages, injuries or losses that may be sustained while in the care of La-ti-Da Music.

• Class availability is subject to sufficient enrollment (at least 5 children)

• Registration will be confirmed by phone or e-mail

• Make-ups may be taken at other class times PLEASE CALL TO SCHEDULE FIRST! (Two makeups allowed per session) Makeups do not carry over.

• Any canceled classes will be rescheduled

• Class schedule is subject to change

Refund policy:

$25 fee if canceling before 1^st class; $50 fee if canceling before 2^nd class; 50% fee if canceling before 3^rd class; no refunds after that.
